ICAM Quick Reference Guide
The ICAM Quick Reference Guide is the field companion to the Incident Cause Analysis Method — a concise, practitioner-focused reference designed to be used alongside live investigations, not stored on a shelf.
The Guide distils decades of applied investigation experience into a structured, step-by-step reference covering the full ICAM process, from initial notification through to final report and corrective action close-out.
What it helps you achieve
What the ICAM QRG App Helps You Achieve
A structured approach to incident investigation helps organisations build capability, improve consistency, and drive better safety outcomes. The ICAM QRG App supports investigators to:
- Establish the facts and circumstances surrounding the operators, task, equipment, location and other relevant conditions
- Identify the contributing factors to the incident and latent hazards involved
- Review the adequacy of existing controls and procedures
- Identify organisational factors that can be analysed by the business or undertaking (BOU)
- Report findings in accordance with organisational protocols
- Recommend corrective actions to reduce the risk of similar incidents occurring
- Identify key learnings for distribution within the business or undertaking
The investigator's toolkit
Every ICAM Tool, Ready in the Field
The ICAM QRG App brings the working tools of an ICAM investigation into a structured digital format — so investigators can capture evidence, plan lines of inquiry, and build their analysis on the same device they carry on site. Tools available in the App include:
- Investigation Plan Template — capture scope, Terms of Reference, team, schedule and logistics
- Team Selection Guidance — when to escalate and when to bring in independence
- PEEPO Plan Template — organise lines of inquiry across People, Environment, Equipment, Procedures and Documents, and Organisation
- Toolkit for Investigation — practical guidance for running the investigation
- Witness Statement Template — structured, defensible interview capture
- Timeline / Sequence of Events — build the factual sequence across the lead-up, the incident, and post-incident recovery
- Investigation Log — a running record of every activity, decision and milestone
- ICAM Analysis Template — the full ICAM code set, structured for capturing contributing factors
- Investigation Report the report structure, ready to populate
Built to Work Alongside ICAM Training
How it fits with training
The ICAM QRG App is a field reference for trained investigators — not a replacement for training or for the methodology itself.
- If you've completed ICAM training, the App is the reference you reach for during a real investigation
- If your organisation uses the ICAM Quick Reference Guide, the App brings that same trusted content into a searchable, structured digital format
- If you're new to ICAM, the App will help you understand the method — but formal ICAM training remains essential before leading a workplace incident investigation
ICAM Australia continues to deliver Lead Investigator training, refresher courses, and on-site investigation facilitation. The App is built to support that training, not substitute for it.

FAQ
Is the App a replacement for ICAM training? No. The App assumes the user is trained in the ICAM methodology. It's a working reference, not a training course. ICAM Australia continues to deliver Lead Investigator and refresher training.
Is it a replacement for the printed Quick Reference Guide? No. The App contains the same content as the Quick Reference Guide in an interactive format. Some teams prefer the printed Guide; the App is for investigators who want it accessible in the field, on the device they already carry.
What devices does it work on? iPhone, Android and desktop web. Sign in once and your access follows you across devices.
How is it priced? A subscription, billed monthly ($24.99 inc GST) or annually ($220 inc GST). You can subscribe from this page.
Can my organisation buy multiple subscriptions? Yes — for team and bulk licensing, contact ICAM Australia directly and we'll arrange the right option for your organisation.
