Our Safety Practitioners and Incident Investigators at ICAM Australia are led by our Principal Investigator, Jo De Landre, who is recognised as one of the most experienced practitioners in the fields of incident / accident investigation, applied human factors and safety management in Australia.

With over 35 years applied experience working in and with an array of industries, Jo has investigated some of the most high profile, significant incidents and accidents in Australia. Jo has trained over 10,000 personnel in incident investigation methodologies and has extensive experience in the conduct of systemic investigations and human factors application and integration.

Jo has post graduate qualifications in Applied Psychology from the Australian National University, is a full member of the International Society of Air Safety Investigators (ISASI), is a full member of and held the position of Secretary of the Australian Aviation Psychology Association (AAvPA) for over a decade and has published many papers in aviation, mining, safety and police journals.

Jo has developed extensive knowledge and applied experience in the following areas:

• Training in incident investigation using various methodologies utilised by clients including Incident Cause Analysis Method (ICAM), 5 Why’s, Reason Model etc.

• Incident Management (Systems, Procedures and Forms) Reviews
• Systemic analysis of occurrence data to identify trends and proactive indicators
• Application and integration of human factors, system safety and organisational behaviour principles to achieve practical human error management solutions
• Risk Management applications including Bow tie analysis
• Compliance and surveillance activities including system and risk audits
• Developing safety management training programs and tools
• Developing industry standards for new equipment / operations
• Health, Safety Environment Management Systems benchmarking and improvement.
• Safety Culture and Just Culture Program Development and Implementation
• Safety Leadership programs
