Collect evidence in a structured way. People, Environment, Equipment, Procedures and Organisation, gathered so it gets used rather than filed.

Interview witnesses. Get an account that is accurate and usable, without leading the person.

Build the timeline and find the factors. Turn statements and records into a sequence, then work from the event to the contributing factors behind it.

Shape recommendations. Contribute actions that address the factors found, not just the last thing that went wrong.
